Ontario Standard Invoice
Company Statement on the Use of the ONTARIO STANDARD INVOICE:

FSCO (Financial Services Commission of Ontario) has approved the requirement, for Ontario, of the Standard Invoice as of November 12, 2001. The Wawanesa Mutual Insurance Company requires the use of the Standard Invoice as set out in FSCO's bulletin. In particular we require that invoices be submitted on the Standard Invoice form with appropriate coded information for services described in FSCO's bulletin. We also expect that billings from any Designated Assessment Centre will be on the Standard Invoice form in accordance with FSCO's requirements. We also require the use of the Standard Invoice with respect to billings for insurer examinations and fees billed to us under section 24 of the SABS.

We do not require the use of the Standard Invoice by treating physicians who are billing only for the completion of a claims form on behalf of a patient.
